Sport Registration (FinalForms)

Belmont Athletics Sports Registration

In order to be eligible to play a NHIAA sanctioned sport, every athlete must complete the following steps. Student athletes must have all 4 pieces of the registration process completed before they will be allowed to participate in a tryout or practice.

We are using FinalForms starting with the school year 2020-2021 to register for athletics.  Contact Information, Medical Information, and Handbook Contracts are all done by going to Belmont Athletics.  Once you set up the information for your student-athlete, that information will stay with them when you sign up for a sport in Winter and/or Spring.

  1. Physical Form.
    1. Physicals are good for 365 days and must be updated every year. Your physical on record cannot expire during your season of play and must be kept on file with the Nurse. Click here for a copy of this form to bring to your doctor.
      1. If your student-athlete's physical is expired, and you have a new physical scheduled but it has not happened yet the school needs a copy of a note from your student-athlete's doctor stating they are approved for sports until their physical or your student-athlete will not be able to try out. The note may be faxed to the school or delivered in person on Monday by your student-athlete.

  2. Impact Test (high school only)
    1. This test is used by the school district to help evaluate head injuries should an athlete sustain one during a school sponsored event. The test is required for athletes every other year and helps us to establish a baseline for use if an injury occurs. Impact test are normally administered at the high school prior to the start of each season. However, with the current situation we are going to do this online this year.
